Insurance Guide

How much will insurance cover?

Helping patients understand reimbursement before treatment begins

  • Based on real reimbursement amounts.
  • Grouped by procedure.
  • We bill your insurance as a courtesy—even though payment is due upfront.
  • We’ve organized examples by procedure, not insurance.
  • Each table shows:
    • Procedure code
    • Our fee
    • The insurance company
    • The amount reimbursed by the insurance.
  • These are real examples, but your reimbursement may vary based on your plan.
Most insurance plans mail reimbursement checks directly to the patient’s address within approximately 15 to 30 business days after the claim is processed.
Please note: Reimbursement times and amounts vary depending on the plan and coverage level.

  • Out-of-network: As out-of-network, you may still have coverage with us; just at a lower rate. Some plans reimburse well, others very little, and some don’t cover outof-network care at all. This usually depends on your plan type, not the insurance company itself.
  • Annual Maximums: Most dental plans have a yearly limit (often $1,000–$2,000). Once that’s used up, insurance won’t pay anything else until the next plan year.
  • Frequency Limitations: Some services (like X-rays or exams) may be denied if your plan only allows them once every 6 or 12 months.
  • Coverage Tiers (commonly referred to as “100-80-50”):
    • 100% Preventive: Cleanings, exams, x-rays.
    • 80% Basic: Fillings, simple extractions.
    • 50% Major: Crowns, bridges, root canals, dentures.
  • Waiting Periods: Some plans require 6–12 months before covering major procedures.
  • Frequency Limits: Services like cleanings or x-rays may be limited to 1–2 times per year.
We’re happy to check your insurance benefits and let you know what may be covered. Our goal is to help you feel informed and confident.
